All praise and thanks are due to Allah (سبحانه و تعالى), and peace and blessings be upon His Messenger (صلى الله عليه و سلم).

Dear questioner,

First of all, we implore Allah (سبحانه و تعالى) to help us serve His cause and render our work for His sake.

Shorter Answer: If the male sexual organ is abnormally short to the extent that it causes harm to the person and makes him unable to function naturally such as where one cannot perform the intercourse in the right way or cannot impregnate his wife, then it becomes permissible to undergo appropriate treatment in consultation with the specialized doctors. However, if the organ is in its natural size and the person wants to enlarge it just out of personal desire, it becomes haraam (unlawful) to do so for two reasons, namely, it involves changing Allah’s Creation without necessity and uncovering one’s `Awrah for non-medical purposes. 

Long Answer: Doctors have determined that it is not a disease for a sexual organ to be small. This is due to the form of creation and people have different sexual organs. However, a sexual organ could be very small and unusual, a matter which could cause a disability or a harm [such as one cannot perform the intercourse in the right way or cannot impregnate his wife for children]. In such a case, there is no harm to undergo a [proper medical treatment] according to the decision of specialized doctors to put the organ back to its normal size. [This could include taking pills, massage or surgical treatment, unless doing so does not cause any harm as] it is permissible to seek treatment because when the Prophet (صلى الله عليه و سلم) was asked by the Bedouins whether or not they are permitted to seek treatment, he replied: “O Slaves of Allah! Seek treatment.” [Abu Dawood and At-Tirmidhi]

Next, Allah (سبحانه و تعالى) says in the Qur’an (referring to the words of Shaytan): “And surely I will command them and they will change Allah’s creation. Whoso chooseth Satan for a patron instead of Allah is verily a loser and his loss is manifest.”  (Soorah an-Nisa’a, 4: 119)

In light of the above ayah of the Qur’an, all the jurists in Islam are unanimous in their opinion that enlarging the penis, just for the sake of enlargement, would be changing the fair nature that Allah has Willed for the person; and thus hold it absolutely impermissible in Islam. (Islamhelpline.net) Also, this can lead him to forbidden things like revealing one’s ‘Awrah (parts of the body that must be covered in Islam) [to others for the purpose of treatment] without any lawful reason.

According to Assim Al-Hakeem, “If it is a defect, it is permissible. But if one has a normal one yet he wants it to be bigger, this is not permissible.”

Sheikh Abdus-Sattar F. Saeed, Professor of the Exegesis of the Qur’an at Al-Azhar University [states]: “If the penis is abnormally short to the extent that it causes harm to the person and makes him unable to function naturally, then it becomes allowable to undergo an operation for the enlargement of his penis. This is considered to be a state of necessity where the person is allowed to do so. However, if the organ is in its natural size and the person wants to enlarge it just out of personal desire for having more sexual gratification, it becomes haraam (unlawful) to do so for two reasons: 1- This involves uncovering the `Awrah (parts of one’s body that should not be exposed in front of others) for non-medical purposes, and 2- It involves changing Allah’s Creation without necessity.”  (Qa.islam.com)
